Save time by completing these forms ahead of time and either faxing them in or bringing them with you for your visit.

Instructions

First, complete the red highlighted fields on the New Intake form (1 page, printable - 110KB, online fillable - 410KB) and fax it to (703) 368-8454 so we can verify your information and insurance coverage as well as enter you into our system.

Once all your information and insurance coverage has been verified, we will contact and instruct you to complete the following Information Packet (8 pages, printable - 30KB). When completed either bring it with you to your first visit or fax it to (540) 658-0855 for Stafford office visits. For visits to our Manassas or Woodbridge offices please complete the package from the Prince William Family Counseling forms web page and either bring it with you to your first visit or fax it to (703) 368-8454.
